Issue

HBA X1147A will not initialize and the card will not be visible in sysconfig -a or sysconfig -ac. Console messages will print the following on the effected port(s).
Initialization of network interface failed due to unexpected software error ixl:43
 
dmes will print the following on the effected port(s) 
[25] e2a: Firmware recovery mode detected. Limiting functionality. Refer to Intel(R) Ethernet Adapters and Devices User Guide for details on firmware recovery mode.
