Issue

  • Drive in DS460C shelf 1, slot 16 reported as faulty on a new DS460-12 installation.
  • Faulty drive was swapped to slot 18 but it failed to come online; but the swapped drive in slot 16 came online.

  • The drive at slot 18 was replaced but it failed to come online.

  • Drive was then moved to slot 52 an empty slot, issue followed the slot.

  • A second replacement drive was installed at slot 52 but that was also bypassed.

  • Drive was moved again to slot 59 — issue persisted; drive remained bypassed on both SAS paths (A and B), making it completely inaccessible to storage controllers

  • Shelf 1 reported CRITICAL enclosure status with drive 59 showing BYPASSED A, BYPASSED B

  • The drive manager was unable to activate the drives in any slot, as confirmed by shelf logs from both IOM A and IOM B modules

  • IOM module ddump logs showed no faults on the IOMs themselves — ruling out IOM hardware failure
