CFBIOS-522: nvme.ssd.attach.error and "ctrlr construct failed" console messages upon disk initialization

Issue

In certain scenarios after inserting an NVMe drive in an AFF A250 system internal drive bay, "nvme.ssd.attach.error" event message with "ctrlr construct failed" appear on the console and ONTAP does not attach the drive.
The following are examples of the messages on the serial console:
Sep 27 17:35:20 [node1:nvme.ssd.attach.error:error]: NVMe SSD encountered error: "ctrlr construct failed" on slot 15. Could not attach drive.
Sep 27 17:35:20 [node1:nvme.ssd.attach.error:error]: NVMe SSD encountered error: "ctrlr construct failed" on slot 8. Could not attach drive.
