 Module type: IOM6; monitoring is active
 Shelf status: unrecoverable
  Voltage Sensor installed element list: 1, 2, 3, 4; with error: 2
  Shelf voltages by element:
    [1] 5.00 Volts  Normal voltage range
    [2] <N/A> sensor condition <N/A>
    [3] 5.00 Volts  Normal voltage range
    [4] 12.03 Volts  Normal voltage range

  • Replaced the PSU-1 (as Voltage Sensor maps in PSU-1), issue exists.
