Cisco Nexus cluster switches shows both PSUs as shutdown

Issue

HealthMonitor process cshm: SwitchPowerFail_Alert [CLUSTER-BOT(FDOxxxxxxxx7)/PowerSupply-1])
  • At the same time the device continues to operate normally even with all the inserted power supplies reported as shutdown. During the issue "OK" LED is seen to be either green or flashing green, and "Fault" LED is seen to be off.
  • Power supplies (including, but not limited to: NXA-PAC-650W-PE or NXA-PAC-650W-PI) could be seen reported as shutdown on the switches as per 
    "show environment power detail" and show environment command.
