Issue

  • Receive the following alerts in EMS log

[Node-01: asd_timeoutd_0a: sas.adapter.firmware.fault:notice]: Detected firmware fault 0x40002628 on SAS adapter 0a.
[Node-01: asd_timeoutd_0a: sas.adapter.exception:debug]: SAS adapter driver encountered error "IOC state returned fault DB: 0x40002628 " on adapter 0a.
[Node-01: asd_asyncd_0a: sas.adapter.reset:debug]: Resetting SAS adapter 0a.
[Node-01: scsi_cmdblk_strthr_admin: scsi.cmd.abortedByHost:error]: Disk device 0a.00.x: Command aborted by host adapter: HA status 0x4: cdb 0x9a:0000000032062938:0001:0028.
[Node-01: scsi_cmdblk_strthr_admin: scsi.cmd.abortedByHost:error]: Disk device 0b.00.x: Command aborted by host adapter: HA status 0x4: cdb 0x9a:0000000032062950:0001:0010.

  • Also observe scsi.cmd.checkcondition errors

[Node-01: scsi.cmd.checkCondition:error]: Disk device 0a.00.x: Check Condition: CDB 0x88:0000000186062d50:00000008: Sense Data SCSI:aborted command -  (0xb - 0x2f 0x10 0x0)(13187).
[Node-01: scsi.cmd.checkCondition:error]: Disk device 0a.00.x: Check Condition: CDB 0x88:0000000182f936c8:00000008: Sense Data SCSI:aborted command -  (0xb - 0x2f 0x10 0x0)(13336).
[Node-01: scsi.cmd.checkCondition:error]: Disk device 0a.00.x: Check Condition: CDB 0x88:000000017c8110f0:00000008: Sense Data SCSI:aborted command -  (0xb - 0x2f 0x10 0x0)(13140).
