Panic String:thread (swi4: clock) on cpu X hung for 4001 milliseconds

Issue

  • The node with the boot device experiencing the issue will panic, similar to:

pass0: CAM_CMD_TIMEOUT, retrying (1 retry left)
aprobe0: CAM_CMD_TIMEOUT, retrying (1 retry left)
ada0 at ahcich0 bus 0 scbus0 target 0 lun 0
ada0:  s/n SPG18280BA5 detached
PANIC  : thread (swi4: clock(X)) on cpu X hung for 4001 milliseconds

  • There may also be additional failure messages after the panic, similar to:

PANIC: thread (swi4: clock (0)) on cpu 0 hung for 4001 milliseconds in process swi4: clock (0) on release 9.7P13 (C) on Sun Jan  2 04:11:47 PST 2022version: 9.4P3: Thu Oct 11 18:25:55 EDT 2018

ERROR: ATA device failure

ERROR: [D9] Boot option loading failed

  • Event logs may indicate unrecoverable IDE device failure on some platforms:
