Disk status is reported as "startup failed" after replacement

Issue

  • After replacing a faulty disk with a new one, the disk status is reported as "startup failed" in sysconfig -a logs:
::> system node run -node <node-name> -command sysconfig -a

  • Disk initialization fails and the following messages are seen in the event logs:

Mon Mar 07 13:18:04 IST [Node-01: pmcsas_asyncd_0: scsi.path.excessiveErrors:error]: Excessive errors encountered by adapter 0a on disk device 0a.00.15.
Mon Mar 07 13:18:04 IST [Node-01: pmcsas_asyncd_0: scsi.cmd.pastTimeToLive:error]: Disk device 0a.00.15: request failed after try #6: cdb 0x1b.
Mon Mar 07 13:18:04 IST [Node-01: pmcsas_asyncd_0: disk.init.failure.error:warning]: Disk 0a.00.15 failed initialization due to error 6.
