OnCommand Unified Manager reports "Cluster Health Poll failed because of Internal error" but Performance and Health Pages show updated data

Issue

In the UM web interface, on the Dashboards > Cluster View page, the following "Cluster Health Poll failed because of internal error" message is seen

The following error is seen within the ocumserver.log file:

[oncommand] [reconcile-1] [cluster(incremental@11:01:05.393)] [c.n.dfm.collector.OcieJmsListener] Warning during inventory change notification: JTA transaction unexpectedly rolled back (maybe due to a timeout); nested exception is javax.transaction.RollbackException: ARJUNA016053: Could not commit transaction.,query did not return a unique result: 2; nested exception is org.hibernate.NonUniqueResultException: query did not return a unique result:

Note: Logs for UM can be accessed in the following locations:

vAPP:

1. Use the How to access ActiveIQ Unified Manager Virtual Machine (OVA) DIAG shell to enable the diag shell

2. Follow the Linux instructions below

Linux (RHEL, CentOS, vAPP):

The ocumserver.log file is located in the /var/log/ocum directory

Windows:

The ocumserver.log file is located in the <install_disk>:\ProgramData\Netapp\OnCommandAppData\ocum\log directory
