Adapter card on FAS9000, AFF A700, and AFF A900 are missing from sysconfig after replacement

Issue

After replacement of an adapter card (such as NIC, SAS, or FC adapter), ONTAP is unable to recognize the card.  It no longer shows as present in the system.
