Node will not boot after ONTAP upgrade

Issue

After a node reboots at the beginning of an ONTAP upgrade, the node will not boot up and shows an error
 
Example:
0 Fibre Channel Storage Adapters found!
...
Target Adapters found:
12 Fibre Channel Target Adapters found!
0 iSCSI Target Adapters found!

WARNING: there do not appear to be any disks attached to the system.
No root volume found.
Rebooting... (press ctrl-c during boot to break reboot loop)
Uptime: 2m29s
System rebooting...
