ONTAP revert on new C-Series cluster fails

Issue

After booting 9.12.1P11 or below on new AFF C800 cluster, ONTAP is unable to recognize the drives and boot.
 
Ex:
 
diskown.errorDuring IO:error]: error 19 (disk not ready for requested operation) on disk 0n.44 (S/N ) while reading individual disk ownership area
