Fault LED lights on shelf without any error

  • Fault LED lights on the shelf.
  • The system is in a mixed path although it's actually a multi-path configuration.
  • There are no errors in the environment.
  • Cascading "expander_phys_state" displays one or more of the PHYs connected to the suspect module as disabled.
