AFF A250 is taken over and cannot boot

Issue

  • Node panics (may or may not be present in the BMC system log console output) like:
PANIC: g_vfs_done(): rootfs.uzip read error - suspect boot device in process g_up on release 9.8P1 (C) on Thu Aug 26 04:05:54 UTC 2021
 
or
 
PANIC  : Process mgwd unresponsive for 159 seconds (mgwd startup: "(2219)")
  • On reboot, the system is stuck at the BIOS and cannot boot past the Aptio Setup Utility
  • The following message is displayed during boot:
ERROR: Boot option loading failed

and

md0.uzip: UZIP(zlib) inflate() failed
g_vfs_done():md0.uzip[READ(offset=360980480, length=8192)]error = 86
vm_fault: pager read error, pid 48638 (gzip) [retry 9]
ending in a PANIC:

  • BMC system log sel indicates watchdog 2 timers interrupt/hard reset:

 264 | 02/03/2022 | 07:04:04 | Watchdog 2 #0x0f | Timer interrupt | Asserted
 265 | 02/03/2022 | 07:05:03 | Watchdog 2 #0x0f | Hard reset | Asserted
 266 | 02/03/2022 | 07:05:05 | Unknown #0x51 | State Asserted
