Sporadically intermittent path loss to LUN

Issue

  • All connected ESXi hosts report sporadically intermittent path loss to FC datastore for 1-2 seconds and then automatically reconnect
  • Repeating EMS log entries about adapter ports receiving reset requests:

  • The portstatsshow output indicates increased er_tx_c3_timeout and er_bad_os (Invalid ordered set)
