ESXi host crash after a storage array controller reboot

Issue

A user might experience an ESXi server panic and enter the Purple Screen of Death (PSOD) after a storage array controller reboot.

Upon PSOD the user will notice that their host is in a disconnected state within vCenter (if applicable). If using a standalone host, the server will be unresponsive, will not respond to packet pings and the web client displays "This site can't be reached" messages.
