SnapCenter does not discover LUNs created outside SnapCenter or VMDK disks

Issue

LUNs created outside SnapCenter or VMDK disks are not listed in the [Hosts] -> [Disks] page.
Even though [Refresh Disks] is clicked on the page, the disks are not discovered.
