Unified Manager OVA boot failure due to file system inconsistency

Issue

  • Unified Manager (UM) GUI is not reachable
  • Within the vSphere Console, UM or OPM virtual appliance fails to boot to the login prompt
  • Upon pressing Alt+F2, the console displays the following message:
fsck failed. Please repair manually....

A maintenance shell will now be started CONTROL-D Will terminate this shell and resume system boot

Give root password for maintenance

(or type Control-D to continue)
  • 'Failed to start File System Check' for a device is shown as 'FAILED' during boot process.
    UM OVA boot failure
  • Failed to mount /dev/sda1 as root file system
