Adapter cards not present when adding expansion cards, error "monitor.ioCard.degraded"

Issue

When adding expansion cards in platforms under the Applies section, cards may not show up in the "sysconfig -ac".  Errors simliar to below are also seen on console and in EMS logs:

Jun 03 19:42:40 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO3 SAS Power Good Power Fault.
Jun 03 19:42:40 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO3 SAS Hot Swap Cur is critical low (0 mA).
Jun 03 19:42:40 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO11 SAS Power Good Power Fault.
Jun 03 19:42:40 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO11 SAS Hot Swap Cur is critical low (0 mA).
Jun 03 19:42:50 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO11 SAS P3.3V is not readable
Jun 03 19:42:50 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO11 SAS P1.8V is not readable
Jun 03 19:42:50 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO11 SAS P1.0V is not readable
Jun 03 19:42:50 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO11 SAS P0.9V is not readable
Jun 03 19:42:50 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO11 SAS 5V Standby is not readable
Jun 03 19:43:11 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO3 SAS P3.3V is not readable
Jun 03 19:43:12 [Cluster-01:monitor.ioCard.degraded:ALERT]: IO card is degraded: IO3 SAS P1.8V is not readable
