Windows cluster disks online but can not read

Issue

Periodically virtual machines (VM) go offline and the Hyper-V host cluster services can not access the CSV where the VM resides. The LUN/Disk is online in windows disk management.
 
Windows error log example:
  • Event ID 1135
  • Cluster validation reports
Node abc holds the SCSI PR on Disk 3 and brought the disk x online, but failed in its attempt to read and validate file data on partition table entry 1. The group or resource is not in the correct state to perform the requested operation.
