OnCommand/ActiveIQ Unified Manager GUI failed to load with ERROR 404 due to bad sectors in the disk

Issue

OnCommand/ActiveIQ Unified Manager GUI failed to load with ERROR 404 and the following error will be observed in dfm-app.war and dfm-report-app.ear files.
