CHAP authentication failure after ONTAP upgrade on ESXi hosts

Issue

Post ONTAP upgrade, iSCSI datastores are showing inaccessible from VMware vCenter.

When connecting to an affected ESXi host, the datastores cannot be accessed.

From ONTAP EMS logs, similar errors can be seen:
