E-Series Controller boots into Grub Rescue screen

Issue

A Controller does not come online.

SSH into the Controller and you are presented with a screen with similar output as below:

error failure reading sector 0x1003 from "HD0"
error can't find command
"pm_map"
error can find command "veos_read"
 
After it boots into a blue menu with the following options:
  • E-series
  • E-Series (persistent)
  • E-Series (persistent debug)
  • E-Series (persistent debug checkpoint)
  • E-Series (persistent debug raid-disabled)
  • E-Series (persistent debug  raid-GDB)
