Issue

  • During planned maintenance  which involves an MCC switchover / switchback an ESXi Host had multiple IO failures on all paths
  • After Metrocluster switchover , fcp initiator show is missing several initiators from hosts
  • igroup show -instance shows Host initiators not logged in
  • Host uses Qlogic 2690
