SAS ADAPTER INITIALIZATION FAILED alert reported in EMS logs

Issue

  • SAS adapter initialization failed error is reported in the EMS logs along with firmware fault and the SAS adapter is taken offline:

Mon Feb 21 14:55:59 +0530 [NODE03: pmcsas_timeout_1: sas.adapter.firmware.fault:notice]: Detected firmware fault 0xffffffff on SAS adapter 0c.
Mon Feb 21 14:56:00 +0530 [NODE03: pmcsas_asyncd_1: sas.adapter.reset:debug]: Resetting SAS adapter 0c.
Mon Feb 21 14:56:02 +0530 [NODE03: pmcsas_asyncd_1: sas.adapter.not.ready:error]: SAS adapter 0c did not become ready.
Mon Feb 21 14:56:02 +0530 [NODE03: pmcsas_asyncd_1: sas.adapter.hardreset:debug]: Hard resetting SAS adapter 0c.
Mon Feb 21 14:56:06 +0530 [NODE03: pmcsas_asyncd_1: sas.adapter.exception:debug]: SAS adapter driver encountered error "Failed to Reset" on adapter 0c.
Mon Feb 21 14:56:06 +0530 [NODE03: pmcsas_asyncd_1: sas.adapter.failed:error]: Failed to recover SAS 0c after multiple resets. Offlining the adapter.
Mon Feb 21 14:56:06 +0530 [NODE03: pmcsas_asyncd_1: callhome.sas.init.failed:error]: Callhome for SAS ADAPTER INITIALIZATION FAILED, Port: .

  • No SAS firmware update was in progress.
  • Observed high workload latency and SAS adapter timeouts. Example:

ERROR scsi.cmd.abortedByHost: Disk device 0c.00.0: Command aborted by host adapter: HA status 0x12: cdb 0x8a:00000001bf178780:00000008.
ERROR sas.adapter.not.ready: SAS adapter 0c did not become ready.
ERROR sas.device.timeout: Adapter 0c encountered a device timeout on Disk device 0c.00.1.
...
