Issue
- SAS adapter initialization failed error is reported in the EMS logs along with firmware fault and the SAS adapter is taken offline:

- No SAS firmware update was in progress.
- Observed high workload latency and SAS adapter timeouts. Example:

ERROR sas.adapter.not.ready: SAS adapter 0c did not become ready.
