Brocade G610/G620 switch fails to boot

The Brocade G610 switch fails to boot with the following console output:

sd 0:0:0:0: [sda] NoCaching mode page found
sd 0:0:0:0: [sda] Assuming drive cache: write through
EXT4-fs (sda1): write access unavailable, cannot proceed
Kernel panic - not syncing: VFS: Unable to mount root fs on unknown-block(8,1)
blk_update_request: critical target error, dev sda, sector 753
Buffer I/O error on dev sda1, logical block 94, lost async page write
blk_update_request: critical target error, dev sda, sector 1497
blk_update_request: critical target error, dev sda, sector 1593
blk_update_request: critical target error, dev sda, sector 2489
EXT4-fs (sda1): error loading journal
Hit ESC to stop autoboot:  5  4  0 
1) Start system.
2) Recover password.
3) Enter command shell.
Option? 3
Boot PROM password has not been set.
