Hyper-V Cluster Shared Volume Offline After Windows Update

  • Microsoft Windows Server 2016 Hyper-V Clusters (iSCSI)
  • Cluster Shared Volumes (CSV) hosted on NetApp LUNs

Issue

  • After applying Windows updates/patches and rebooting a two-node Windows Server 2016 Hyper-V cluster, the Cluster Shared Volume (CSV), backed up by LUN hosted on NetApp storage, could not be brought online. The affected disk displayed as offline in Windows Failover Cluster Manager and Disk Management.
 
  • Observed error messages and log output:
Failover Cluster Manager:
Reserved (The disk is offline because of policy set by an administrator)
The group or resource is not in the correct state to perform the requested operation.
 
Event Log and PowerShell:
Node <hostname> holds the SCSI PR on <disk>, brought disk online, but failed to write file data
to partition table entry 1.
SCSI-3 reservation errors, “Disk offline due to policy,” and persistent reservation not cleared.
