ESXi pathing degradation with iSCSI and LACP

Issue

In an environment utilizing iSCSI and interface groups with LACP enabled on the switch, the esxi hosts are reporting near constant pathing loss and pathing regained events happening on their hosts along with heartbeat timeouts.
Lost access to volume 69456395-6af690f5-a6e8-405b7f3945a0 (AFF_A_ISCSI01) due to connectivity issues. Recovery attempt is in progress and outcome will be reported shortly.

On the storage array, no errors are present on the individual ethernet ports, however, there is a near constant flow of initiators logging back into storage, even though a session already exists:
