fcp.io.status:debug STIO Adapter found hung 
  • Multiple FC hosts may report Partially connected yet show initiators logged in
    igroup show -v 

  • New initiator connections report REJECT from storage target yet show initiators logged in
  • All Host paths may report dead until the target port / switch port is reset
