Error connecting LUN to host - Failed to connect disk No disk is found on all nodes

Issue

Not possible  to connect disks or to create disks on existing hosts:
 
  • When trying to connect to an existing LUN with an existing host (with just LUNs mapped):

Failed - Failed to connect disk No disk is found on all nodes

  • When trying to connect to an existing LUN with a new host (no LUNs mapped):

Failed - Failed to create windows file system Failed to get disk information [81OX-+XQ7two]. Please check if there is SAN connectivity between the host/hypervisor and storage controller
