Trident: Unable to mount volume on Pod with "iSCSI login error: exit status 6"

Issue

When creating a POD with PVC to provision persistent volumes on SolidFire storage cluster using Trident, the PV successfully created but fails to attach with the following errors on the POD Events:

And when trying to manually mount the volume from the worker nodes using iscsiadm it fails with the following error:
