Issue

Windows host with connected LUN saw the following events regarding the Disk going offline:

Example:
"disk1 has reached a logical block provisioning permanent resource exhaustion condition" and "The volume netapp-Lun1 was not optimized because an error was encountered. The disk was disconnected from the system".
