FC initiator port offline due to faulty SFP

Issue
- FC initiator port connected to tape storage through a Cisco switch is offline as seen in
SYSCONFIG-A
orFC-INIT-PORT-INFO.XML

- Link break seen in EMS:

- Upon reseating or swapping the cable, the adapter comes online for a few seconds then goes offline again:
