SAN host reports disk device failure when through uplink switch path

For example:

  1. SAN host only connects to switch01 by FCP protocol
  2. Switch01 connects to the uplink switch02 by ISL(inter-switch link)
  3. NetApp storage connects to both switch 01 and switch02 by different FC adapters
  4. SAN host reports disk (NetApp LUNs) failure error intermittently when accessing the LUNs through the uplink switch (SAN host --> switch01-->switch02-->NetApp)
