ONTAP Upgrade fails with error: Duplicate initiators are mapped to the same LUN

Issue

Upgrade might fail with errors about being unable to normalize initiator names when upgrading ONTAP on a cluster with initiators that contain upper case characters. 

After a node reboots on the newer ONTAP version, the ANDU pauses due to the upgrade not completing(for example):
cluster1::> version
NetApp Release 9.9.1P6: Tue Dec 21 13:38:22 UTC 2021

Info: The output from the version command above may not be correct because 
      upgrade is in progress or has failed in one or more nodes in the cluster.
      Use the "upgrade-revert show" command in advanced mode to view the status
      of upgrade.

Node: cluster1-01                                  Status: 
                                                              aborted

Status Message: The upgrade aborted. Contact support personnel for the upgrade repair procedure.

2200 post-apps  aborted  Duplicate initiators are mapped to the same LUN. Do not try to fix this problem. Using either the 'lun mapping delete' command or the 'lun igroup remove' command could lead to an outage. Contact technical support for assistance.
