Unexpected controller down with console message "Failed to write into flash"

Issue

  • Node unexpectedly down and initiated takeover from partner
  • After power cycling the controller, the system stops after reporting during boot:

Failed to write into flash at offset=0x123a0000 in loop after retries (5)... Destage DataCopy Failed."

  • Powering off and on and the system reports the following message and enters into the BIOS menu:

Boot option loading failed

  • The issue remains after a controller re-seat
  • The issue remains after a boot device replacement
