Single path connected controller repeats rebooting with SAS port offline

Issue
Controller boot failure with below errors:
[localhost:disk.checksum.offlineAdapter:error]: Adapter 0a taken offline due to checksum verification failure on multiple disks.
If the SAS adapter is offlined, the following additional errors will be seen:

0 Fibre Channel Storage Adapters found!
2 SAS Adapters found!

Select option 4 from the boot menu to choose disks for the root volume and spare core space.

No root volume found.
