Offline SAS host adapter and all phys are disabled due to faulty SAS adapter card

Issue

  • SAS host adapter on all the nodes shows offline and all the phys of the ports shows disabled
  • SAS card firmware and other details are not recoginized by the system
  •  sysconfig -a  example:

WARNING

Takeover of the node is needed to replace/reseat the SAS adapter card, and followed by giveback once reseat or replace is done.
Make sure hosts has redundant connections with the nodes and MPIO is in place.
