MDP caused by disk reconstruct and continued medium errors on X425_HCBEP1T2A10

Issue

Disk are encountering continued medium errors causing them to go into test/ reconstruct.
  • If multiple disk start hitting this issue around the same time it can cause MDPs (Multi-disk Panics).
  • Upon reboot the disk having issue are marked good but then begins to go into reconstruct and the process loops.
