MDP caused by disk reconstruct and continued medium errors on X425_HCBEP1T2A10

Issue

Disk are being hit by continued medium errors causing them to go into test/ reconstruct.
  • If multiple disk start hitting this issue it can cause MDPs.
  • Upon reboot the disk having issue are marked good but then begins to go into reconstruct and the process loops.
