EMULEX Adapter Showing a "?" in Switch Port Field After SFP Replacement

Issue

  • Post SFP replacement or hardware swap, the adapter is stuck in a hung state and does not allow connection despite being Online/UP.
  • This can appear as a ? in the Switch Port field for initiator ports:
