A disk been reported as failed, accompanied by the serial number of a shelf

Issue

- Following a DC maintenance operation, one disk was indicated as failed from both paths of a node.
  XXXX-FAB1-SW3:32.126L25   : 0.0GB 0B/sect (Startup failed.)
  XXXX-FAB2-SW4:32.126L25   : 0.0GB 0B/sect (Startup failed.)

- The takeover/giveback of the node will not resolve the problem.
- Rebooting ATTO bridge will not rectify the problem.
- The SASTargets output from the ATTO bridge includes a list of 96 drives (numbered 0-95 across 24 shelves with 4 drives each); however, one entry identified as a disk corresponds to the serial number of a shelf.
