E-Series Volume Keeps Going Offline: iSCSI Carrier Lost on Controller Ports

Issue

Production volume (~perf01) intermittently goes offline, causing all ESXi hosts to lose access to data. The following log and symptom details were observed:

  • Log output:
    • “port on both controller A and B are reporting ISCSI carrier lost. Then it is discovered and restarted. But the process repeats over and over.”
    • “Port statistics show receiving errors but no transmitted errors.”
    • “Volume fails over between controllers due to IO shipping triggered by host inability to communicate with the preferred path.”
