SCW fails to clone or provision disk when there is no active ISCSI session

SnapCenter Plug-In for Windows (SCW)

Issue

When attempting to provision or clone a disk with SnapCenter Plug-In for Windows (PowerShell or GUI) the below error message is generated in the SCWDebug.log

2020-12-28T14:45:57.6085741-08:00 Verbose SDW PID=[17180] TID=[18188] Failed to get disk information [<serial number>]. 
Please check if there is SAN connectivity between the host/hypervisor and storage controller.
2020-12-28T14:45:57.6085741-08:00 Verbose SDW PID=[17180] TID=[18188] Did not find the correct disk
