SnapCenter fails to discover SCSQL host disks, but shows SQL databases

Issue

SnapCenter is able to discover the SQL databases on a host, but fails to discover the disks on the host.
 
Errors seen in  SnapManagerWeb_<Job_Id>.log:
 
ERROR SnapManagerWeb_<Job_id> PID=[<PID>] TID=[<TID>] WebException caught while invoking https://<SC_Server_FQDN>:8145/SMCoreHostManagementService/DiscoverResources
ERROR SnapManagerWeb_<Job_id> PID=[<PID>] TID=[<TID>] WebException Encountered, Response null
ERROR SnapManagerWeb_<Job_id> PID=[<PID>] TID=[<TID>] Exception in method: InvokeXML.
System.Net.WebException: The underlying connection was closed: An unexpected error occurred on a receive. ---> System.IO.IOException: Unable to read data from the transport connection: An existing connection was forcibly closed by the remote host. ---> System.Net.Sockets.SocketException: An existing connection was forcibly closed by the remote host
