ONTAP Tools Services deployment has failed with error code 05061 due to crashed vault service

• After logging into OTV web console to check the status via Option 1, the Appliance status output is blank
• ONTAP tools status is showing Not Running
• The OTV VM may have been found in this state while powered ON or in a powered-off state, possibly for multiple days
• Customers may notice that their OTV discovery tasks are failing
• After checking the OTV VM's via Web Console, it shows an error 05061
• When trying to run commands from the diagnostics shell, the following may be displayed:

E0626 21:38:57.870066 2339246 memcache.go:238] couldn't get current server API group list: the server has asked for the client to provide credentials

• Powering on or rebooting the ONTAP Tools for VMware vSphere (OTV) VM fails with the same 05061 error code.
  • Note: 05061 is a deployment error code that indicates a problem with OTV services.  For more information regarding error codes, please see Deployment error codes
• From the OTV VM CLI, the kn get pods output shows the ntv-vault-0 pod is in a CrashLoopBackOff state

• All OTV plug-in operations are not available
• Menu > NetApp ONTAP tools displays no healthy upstream on the web user interface of VMware vSphere
• When shutting down the OTV VM, errors are repeatedly returned
