OpenStack Integration with NetApp ONTAP: LUN Creation Fails with “LUN already mapped” and Multiple Stale igroups

Issue

When attempting to create or attach a volume to a VM in OpenStack using NetApp ONTAP as the backend, the operation fails. Multiple stale igroups are created on ONTAP, and subsequent attempts continue to fail with the following error:

Log Output / Error Message:

Build of instance <UUID> aborted: Unable to update attachment (Bad or unexpected response from the storage volume backend API: Driver initialize_connection failed (error: NetApp API failed. Reason - 1254223: LUN already mapped)

ONTAP CLI shows multiple igroups with the same initiators:

::> igroup show -vserver SVM_Name -igroup openstack-*
...
12 entries were displayed.
