Trident gives error "persistentvolume-controller waiting for a volume to be created"

Issue

Trident installed with no errors, but will not create a new volume.

In the trident-controller log you see messages similar to:

[10:12:14.166952 reflector.go:156] pkg/mod/k8s.io/client-go@v0.17.0/tools/cache/reflector.go:108: Failed to list *v1.TridentTransaction: Get "https://172.30.0.1:443/apis/trident.netapp.io/v1/namespaces/trident2/tridenttransactions?limit=500&resourceVersion=17074569": dial tcp 172.30.0.1:443: connect: connection refused
