Trident ONTAP-SAN driver cannot find multipath device after node upgrades, failing PV

Issue

After a node upgrade, or on a fresh Kubernetes setup iSCSI PVs stop being mounted after LUN creation on certain nodes.
 
The following message is found in the trident logs:
