StorageGRID upgrade leaves StorageGRID Appliance storage node stuck in maintenance mode

Issue

When a node rebooted during an upgrade, the upgrade screen shows the node staying in a status of rebooting.

When verifing the logs on the StorageGRID Appliance, it shows:

/dev/mapper is empty
[2020-10-17 14:38:56+00:00 SGA] WARNING -- Not all volumes are present in /dev/mapper CHECK MULTIPATH CONFIG
[2020-10-17 14:38:56+00:00 SGA] ERROR -- Mappings do not exist!
 
When the container is looking for the StorageGRID OS LUNs, its unable to find it so it sets up the node as a new installation.
 
[2020-10-17 14:38:56+00:00 SGA] [2020-10-17T14:38:56.852] - INFO -- Not loading synced data from SG OS LUN. Device /dev/mapper/StorageGRID-SG-OS-part1 does not exist
 
From SANtricity (E-Series Controller), a host path is seen as down while both should be in use.
