Disk not recognized on both paths by one node of an HA-Pair

Issue

• Disk missing on both paths for one node, but not the other.
• Hard resets to the disk in both node's adapters. EMS errors example:

• A disk re-seat and replace didn't solve the isssue
• An IOM re-seat and replace didn't solve the issue.
